ABSTRACT

Notch signaling pathway is a highly conserved signaling pathway in evolution, which would be activated to play a role in apoptosis, angiogenesis, neurogenesis, immune inflammation and others after ischemia. After acupuncture for ischemic stroke, the expression of Notch signaling pathways related factors increase, which may play a role in neurogenesis, ischemic tolerance, apoptosis, angiogenesis, and so on.

ABSTRACT

Netrin-1 may protect and repair the damage caused by cerebral infarction, in terms of inhibiting apoptosis and inflammatory, and promoting angiogenesis and axon regeneration, etc. Netrin-1 may associate with the pathogenesis and outcome of cerebral infarction. The application of Netrin-1 in clinic needs more researches.

ABSTRACT

OBJECTIVE@#To observe the clinical effect of ' -- needling technique for diabetic fundus hemorrhage.@*METHODS@#With before-after study design, 34 patients with diabetic eyeground hemorrhage were treated with basic treatment (oral administration of antidiabetic medication or insulin injections to ensure blood glucose in the normal range); in addition, acupuncture was given at bilateral Fengchi (GB 20), Taiyang (EX-HN 5), Jingming (BL 1), Cuanzhu (BL 2), Sanyinjiao (SP 6) and Hegu (LI 4). The ' -- needling technique was applied at Fengchi (GB 20); the heat reinforcing needling technique was applied at Taiyang (EX-HN 5); the slow needle insertion technique was applied at Jingming (BL 1); and the - needling technique was applied at Cuanzhu (BL 2); the neutral supplementation and draining method was applied at remaining acupoints. The acupuncture was given once a day, 6 times as one course, and totally 4 courses were given with an interval of 1 day between courses. The follow-up visit was 6 months after treatment. The TCM symptom scores, fundus examination results and vision improvement were observed before and after treatment, and the effect was observed.@*RESULTS@#Compared before treatment, the visual acuity, TCM symptom scores, fundus microaneurysm and hemorrhage points in 34 patients (68 eyes) were significantly improved after treatment (0.05).@*CONCLUSION@#' -- needling technique could improve symptoms, promote the absorption of fundus hemorrhage, and improve vision in patients with diabetic fundus hemorrhage.

ABSTRACT

Background@#Anastomotic leakage is a serious surgical complication in rectal cancer; however, effective evaluation methods for predicting anastomotic leakage individual risk in patients are not currently available. This study aimed to develop a method to evaluate the risk of leakage during surgery.@*Methods@#The 163 patients with rectal cancer, who had undergone anterior resection and low-ligation procedures for Doppler sonographic hemodynamic measurement from April 2011 to January 2015 in Peking University Cancer Hospital, were prospectively recruited. A predictive model was constructed based on the associations between anastomotic leakage and alterations in the anastomotic blood supply in the patients, using both univariate and multivariate statistical analyses, as well as diagnostic methodology evaluation, including Chi-square test, logistic regression model, and receiver operating characteristic curve.@*Results@#The overall anastomotic leakage incidence was 9.2% (15/163). Doppler hemodynamic parameters whose reduction was significantly associated with anastomotic leakage were peak systolic velocity, pulsatility index, and resistance index. The areas under the receiver operating characteristic curve of residual rates of peak systolic velocity, pulsatility index, and resistance index in predicting anastomotic leakage were 0.703 (95% confidence interval [CI]: 0.552–0.854), 0.729 (95% CI: 0.579–0.879), and 0.689 (95% CI: 0.522–0.856), respectively. The predictive model revealed that the patients with severely reduced blood-flow signal exhibited a significantly higher incidence rate of anastomotic leakage than those with sufficient blood supply (19.6% vs. 3.7%, P = 0.003), particularly the patients with low rectal cancer (25.9% vs. 3.9%, P = 0.007) and those receiving neoadjuvant chemoradiotherapy (32.1% vs. 3.7%, P = 0.001), independent of prophylactic ileostoma. Multivariate analysis revealed that insufficient blood supply of the anastomotic bowel was an independent risk factor for anastomotic leakage (odds ratio: 10.37, 95% CI: 2.703–42.735, P = 0.001).@*Conclusion@#Based on this explorative study, Doppler sonographic hemodynamic measurement of the anastomotic bowel presented potential value in predicting anastomotic leakage.

ABSTRACT

OBJECTIVE@#To identify the effectiveness of auricular acupressure (AA) in patients with acute postoperative pain after surgery by systematic review.@*METHODS@#A search of randomized controlled trials was conducted in 5 English medical electronic databases and 4 Chinese databases. Two reviewers independently retrieved related studies, assessed the methodological quality, and extracted data with a standardized data form. Meta-analyses were performed using all time-points meta-analysis.@*RESULTS@#A total of 26 studies with 1,682 participants were included. Results showed that compared with conventional therapy, AA significantly improved the total effective rate [risk ratio=1.25, 95% confidence interval (CI), 1.13 to 1.37, Plt;0.0001; heterogeneity: Plt;0.0001, I=85%]. In the subgroup analysis, the results changed in different follow-up time and surgery categories. The pain relief in the AA group might be the most significant at 72 h after surgery (mean difference=-0.85, 95% CI,-1.20 to-0.50, Plt;0.0001) and in abdominal surgery (mean difference=-1.15, 95% CI,-1.41 to-0.90, Plt;0.0001). Sensitivity analysis demonstrated that the results of this meta-analysis were stable. No serious adverse effects were recorded.@*CONCLUSION@#It was recommended to provide AA to patients with acute postoperative pain. However, a more accurate estimate of the effect requires further rigorously designed large-scale and high-quality RCTs for improving acute postoperative pain after surgery.

ABSTRACT

This article is based on the possible common mechanism of pain and depression. Recent 10 years' reports on basic and clinical studies related to acupuncture treatment for pain and depression have been retrieved to explain that acupuncture can stop pain from many aspects to relieve depression. It is hoped that acupuncture can become a green and safe alternative therapy, reduce the toxic side-effects of drugs, and be popularized and applied to clinical treatment.

ABSTRACT

Objective To establish nursing prevention strategies for patients with deep venous thrombosis in department of orthopedics,so as to guide the specific context of clinical nursing practice,offer evidence-based evidence.Methods Clinical practice guidelines and original researches at home and abroad were retrieved using systematic review approach,and appraisal of literatures was conducted using Australia JBI based Quality Evaluation Center for health care evaluation methods and Johns Hopkins standard.The preliminary nursing prevention strategies for patients with deep venous thrombosis in department of orthopedics and Delphi mail questionnaire were developed.Using Delphi method,the operational feasibility of the final evidence strategies was improved for clinical evidence application through two rounds of consultation from clinical experts.Results A total of 111 articles were included in this study.Forty-nine prevention strategies were established under 3 dimensions including "risk assessment graded prevention" and "education and instruction" through systematic review and Delphi method.Conclusion Nursing prevention strategies in department of orthopedics for patients with deep venous thrombosis based on systematic review and Delphi method have scientific practicability and operability,and can provide objective reference and implementation strategies for nursing practice,assisting the implementation of effective preventive measures.

ABSTRACT

Objective By comparing the accuracy of different multi-rigid-body models used for simulating walking process of elderly women, to explore the effect of walking speed on the load of knee joints based on the obtained optimal model. Methods In human motion simulation software ADAMS/LifeMOD, the individualized human body models with 19 (M1), 16 (M2) or 12 (M3) links and the corresponding grounds were established, respectively. Then, the dynamic simulation of gait based on 3 models was conducted in turn. Results By comparing the vertical ground reaction force (vGRF), the walking time, the lower extremity joint angles among 3 models, M2 was the most applicable model to reproduce the real performance of gait. When elderly woman fastened their walking speed, the peak values of vGRF, the knee joint torque and power peak were all increased significantly. Conclusions It is suggested that elderly women should do more training for their quadriceps to improve their walking behavior. The research findings also provide references for rehabilitation treatment of knee osteoarthritis patients in clinic.

ABSTRACT

Hypertension is one of main risk factors for the occurrence and death of stroke and coronary heart disease. Its prevalence rate is rising year by year. It severely threatens the health of the human beings. The acupuncture method of "activating blood and dispersing wind, harmonizing Gan-Pi" for treating hypertension launched by Academician SHI Xue-min has aroused great attention due to good cur- ative effect and less adverse reactions. In this paper principles of the circular motion covered by the acupuncture method of "activating blood and dispersing wind, harmonizing Gan-Pi" were clarified.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effect of chronic fatigue syndrome (CFS) treated with moxibustion at Gaohuang (BL 43).</p><p><b>METHODS</b>With stratified block randomization, 72 patients accorded with inclusive criteria were divided into a moxibustion at Gaohuang (BL 43) group (moxibustion group) and an acupuncture group, 36 cases in each one. In the moxibustion group, Gaohuang (BL 43) was treated with big moxa cones as the main acupoint, 10 cones a time; Qihai (CV 6) and Zusanli (ST 36) were added with big moxa cones, 7 cones a time. In the acupuncture group, acupoints were the same as those in the moxibustion group, and twirling reinforcing method was used after qi arriving, 60 times one minute and 360° with range. In the two groups, 10-day treatment was made into one course and there were two days between courses. The treatment was given once a day for 3 courses. Changes of fatigue assessment index (FAI) before and after treatment and clinical effects were observed.</p><p><b>RESULTS</b>The total effective rate was 88.9% (32/36) in the moxibustion group, which was better than 72.2% (26/36) in the acupuncture group apparently (P < 0.05). After treatment in the two groups, FAI scores were obviously declined compared with those before treatment (both P < 0.01) and FAI score in the moxibustion group was apparently lower than that in the acupuncture group (P < 0.05).</p><p><b>CONCLUSION</b>Moxibustion at Gaohuang (BL 43) can improve the FAI score of patients with CFS and the clinical efficacy is definite.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effects of blood activating wind dissipating acupuncture (BAWDA) on blood pressure (BP) of prehypertension (PHT) patients.</p><p><b>METHODS</b>Totally 60 PHT patients were assigned to the control group and the acupuncture group according to random digit table, 30 in each group. All patients were intervened by life style. BAWDA was additionally performed in patients in the acupuncture group for 6 weeks (30 times). The improvement of BP after intervened by acupuncture was observed. BP success rates and the proportion of PHT progressing to hypertension (HT) were also observed after 6-week intervention of acupuncture and at 1-year follow-up.</p><p><b>RESULTS</b>Systolic blood pressure (SBP) and diastolic blood pressure (DBP) decreased after 6-week intervention in the acupuncture. The BP control rate was 56.7% (17/30 cases) in the acupuncture group vs.10.0% (3/30 cases) in the control group with statistical difference (chi2 = 14.70, P < 0.01). At 1-year follow-up BP success rate was 36.7% (11/30 cases) in the acupuncture group, remarkably higher than that of the control group [13.3%, (4/30 cases)] (chi2 = 4.36, P < 0.05).</p><p><b>CONCLUSIONS</b>BAWDA showed BP regulating roles in a gradually stable decreasing tendency. It also could elevate BP success rate of PHT, and reduce the risk of PHT progressing to HT.</p>

Aiming at preventing damages of the target organs, improving the living quality of patients, and avoiding cardio-cerebral accidents, Dr. SHI set up his own methods to treat hypertension. Based on the theory of "sea of qi" and "mental regulation", he adopts Renying (ST 9) as the major acupoint, eliminating wind through activating blood circulation and regulating the function of liver and spleen are taken as the treating principles. Acupuncture manipulations are strictly regulated with clear criterions. Summarizations on pathological basis and clinical researches on hypertension treatment with acupuncture are carried out as well.

ABSTRACT

ObjectiveTo investigate the characteristics of autonomic nervous response of armymen with different mentalqualityuse the enhanced emotion regulationstrategies during fear emotion processes.Methods Mental Quality Questionnaire of Armymen was used to select participants from 818 armymen,screening out 30 armymen with low mental quality,30 armymen with moderate and 30 armymen with high.With elicitation of fear emotion,the participants adopted enhance emotion regulation strategies (cognition emphasis and emotion catharsis),and their blood volume pulse( BVP),skin conductance( SC ),normalized high frequency spectral component(HFnorm),standard deviation of all normal intervals(SDNN) and the recovery time of BVP and SC were recorded.Results ( 1 ) The different mental quality of army men activity in the autonomic physiological changes were not significant differences in resting state.(2) When using the cognition emphasis,the SC of different mental quality of armymen were reduced,and low mental quality of armymen( ( -0.67 ± 1.05 )μ.Ω),moderate mental quality of armymen ( ( - 0.49 ± 0.75 ) μΩ) had larger SC than high mental quality of armymen ( ( - 2.16 ± 2.18 ) μΩ ) ;high mental quality of armymen( ( - 2.17 ± 8.96) nU ) had lower HFnorm than the control( ( 6.96 ± 7.08 ) nU ).When using emotion catharsis,high mental quality of armymen ( ( 1.95 ± 6.58 ) nU ) had lower HFnorm than the control( (6.96 ±7.08)nU).(3)When using the cognition emphasis,low mental quality of armymen ((65.50 ±43.43 )s) had longer recovery time of BVP than high mental quality of armymen( (26.50 ± 25.72)s).When using emotion catharsis,low mental quality of armymen( (71.50 ± 48.36)mm Hg)had longer recovery time of BVP than high mental quality of armymen ( ( 21.00 ± 35.50 ) mm Hg) ; low mental quality of armymen ( ( 103.50 ± 38.01 ) s)had longer SC than moderate( (63.50 ±48.59)s) and high mental quality of armymen( (61.50 ±33.09) s).Conclusion The ability to regulate sympathetic in low mental quality group is weaker than the high mental quality group,although sympathetic can be suppressed when enhanced emotion regulation are adopted.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the immediate effect and the long-term effect on blood pressure (BP) in acupuncture for hypertension.</p><p><b>METHODS</b>Seventy cases of primary hypertension were acupunctured at Renying (ST 9), Hegu (LI 4), Taichong (LR 3), Quchi (LI 11) and Zusanli (ST 36) at 2:00-4:00 pm. BP was recorded before treatment and 5 min, 15 min and 30 min after acupuncture separately every day. The treatment was given for 5 days each week and stopped for 2 days. Totally, 3-month treatment was required. BP was compared at each time point with that before treatment separately on the same day, and made the comparison of it before the treatment every 15 days with that before acupuncture on the 1st day.</p><p><b>RESULTS</b>On the same day, the systolic pressure at each time point after acupuncture was decreased as compared with that before acupuncture, indicating the statistically significant difference (all P < 0.05). The diastolic pressures at each time point on the 1st, 15th, 30th and 60th days of acupuncture were decreased as compared with that before acupuncture, indicating the statistically significant difference (all P < 0.05). In the 3-month treatment, the systolic pressure and diastolic pressure before treatment every 15 days were decreased as compared with that before the treatment on the 1st day (all P < 0.05).</p><p><b>CONCLUSION</b>Acupuncture can decrease the immediate BP and maintain the anti-hypertensive effect for primary hypertension so as to achieve the long-term stable anti-hypertensive effect.</p>

ABSTRACT

ObjectiveTo study the effect of adaptive group psychological training on mental quality and emotion of soldiers.MethodsA total of 48 soldiers ( experiment group) received group psychological training and read a textbook,Mental Quality Training for soldiers,for 3 months,and the other 45 soldiers served as control group.Armyman mental quality inventory (AMQI),self-rating depression scale (SDS) and state-trait anxiety inventory(STAI) were employed to evaluate the mental quality and emotion of soldiers before and after training.All data were analyzed by paired-sample t test.Results ①As compared with pre-training,the AMQI scores of training group increased significantly in bravery ( 71.50 ± 9.59 ),self-confidence ( 68.24 ± 8.36 ),aptitude ( 74.54 ± 10.81 ),loyalty(75.29 ± 8.08 ) and general score ( 359.07 ± 20.30 ) (P < 0.05,P < 0.01 ) ; and no significant difference were found in those of control group(P > 0.05 ).②When compared with pre-training,the scores of SDS (39.00 ±8.38),SAI(40.14 ±9.92),TAI(41.16 ±7.47) and STAI(81.30 ± 12.52) of training group decreased significantly (P < 0.05,P< 0.01 ) ; and no significant difference was found in those of control group(P >0.05 ).ConclusionGroup psychological training can improve mental quality of soldiers,which is helpful to relax anxiety and depression of soldiers.

ABSTRACT

<p><b>OBJECT</b>To explore the better treatment of lumbar intervertebral disc herniation.</p><p><b>METHODS</b>One hundred and thirty-two cases of lumbar intervertebral disc herniation were randomly divided into an observation group and a control group, sixty-six cases in each group. In observation group, Baihui (GV 20) penetrating to Taiyang (EX-HN 5), Jiaji (EX-B 2), Huantiao (GB 30) and Chengfu (BL 36) etc. on affect side were applied; in control group, Shenshu (BL 23), Dachangshu (BL 25), Huantiao (GB 30) and Chengfu (BL 36) etc. on affect side were applied. Once a day, 6 treatments made one session and one day break between courses. The curative effects after 4 and 8 courses and the recurrence rate that 6 months after treatment were observed; the pain scale evaluated by Visual Analogue Scale (VAS) and the Oswestry Disability Index (ODI) were observed.</p><p><b>RESULTS</b>The cured rate and total effective rate were 44.3% (27/61) and 100.0% (61/61) in observation group, 26.7% (16/60) and 93.3% (56/60) in control group after 4 courses; there were significant differences between two groups (all P < 0.05). After 8 courses, the cured rate and total effective rate were no significant differences between two groups (all P > 0.05). In observation group, the recurrence rate was 9.4% (5/53) 6 months after treatment, which was lower than that (24.1%,13/54) in control group (P < 0.05). The VAS and ODI scale were significantly reduced (all P < 0.05) after treatment, and which in observation group were inferior to those in control group (all P < 0.01).</p><p><b>CONCLUSION</b>The lumbar intervertebral disc herniation treated with puncture by opening meridian and consolidating origin is rapidly effected with low recurrence rate.</p>

ABSTRACT

ObjectiveTo investigate psychological trouble of freshmen and their needs for mental health services in military medical university. MethodsAn half-open questionnaire was employed to evaluate 854 freshmen by group-style in military medical university.Results①Freshmen were most concemed about their training stress,soldier role adaptation and emotional trouble,etc.As compared with the male,the rate of emotional trouble and interpersonal conflict of female were increased obviously ( P<0.05 ).②The most important factor resulting in psychological problems was poor self-control,learning and training stress,military management,and so on.③The most attracting style of mental health services was communication with psychological personnel,psychological manual and movie,psychological lectures,and so on.As compared with the male,the rate of female were increased in individual counseling obviously ( P<0.01 ).④Freshmen most focused on education of general psychological knowledge,emotion and stress management,soldier role adaptation,and so on.As compared with the male,the rate of female were increased in emotion and stress management,decreased in service role adaptation obviously ( P<0.05 ).⑤Freshmen were most interested in emotion management,self-awareness,soldier role adaptation,and so on.As compared with the male,the rate of female were increased in emotion management training and profession programming,decreased in soldier role adaptation obviously ( P<0.05 ).Conclusion Freshmen's psychological problems and needs for mental health services showed obvious gender difference.Military mental health services should be focused on development objectives,specification of ser-vices ,andimprovement of the service availability in military medical university.
